No child will be permitted to participate in practice without having both completed.


Also if anyone is looking to volunteer to be chain gang, team mom, monitor, etc.MUSTfill out the 2010 Volunteer Aplication, provide a copy of your NJ drivers license (front and back), and must provide an original Social Security Card (for verification).

Below please find a list of important dates for August

8/1 Pre season begins - please report to Roggy field @ 4:45 p.m. If you have outstanding paperwork please arrive at 4:30.  Anyone who is missing paperwork will not be able to participate in the kickoff practice and will be unable to participate in any scrimmages on 8/13,8/14, 8/15.  IT IS IMPORTANT TO NOTE ALL MEDICAL FORMS MUST BE STAMPED AND SIGNED BY YOUR PHYSCIAN OTHERWISE IT IS INCOMPLETE AND YOUR CHILD WILL NOT BE ABLE TO PARTICIPATE.

ALL PARTICIPANTS WILL NEED THE FOLLOWING PAPER WORK TO PARTICIPATE IN THE AUGUST 1ST PRACTICE: (football- if your child does not participate in the 8/1 practice he will be inelligble for scrimmages or bowl games scheduled for 8/14, or 8/15).

1. White Card with Current Picture (new picture every 2 years) - the entire front of this card needs to be filled out.     If you need a white card, please contact your team mom  Please submit a picture if your picture hasn't been updated in 2 years.

2. Medical Form filled out by physician in 2010, SIGNED and STAMPED - also signed by parent/guardian - http://jspwfl.org/2010PhysicalForm.pdf

3. Parental Consent/Participant Contract - http://jspwfl.org/2010playercontract.pdf

4. 2009-2010 FINAL REPORT CARD (3 copies please).

5. Proof of Insurance (a copy front and back of your insurance card).  

6. Original Birth Certificate and Copy - new players only.  For all new players, you will need to physically show your original birth certificate with raised seal and submit a copy for us to keep in book.

Please be informed your team mom will be contacting you to make arrangement to receive you child’s appropriate paperwork.  If you child’s paperwork is not received prior to Sunday August 1st you child will not be able to participate in the first practice as well as the quad scrimmages and mitey mite days scheduled for 8/14, or 8/15.

Pop Warner National mandates all participants to have 10 hrs of non pad conditioning and 10 hrs of pads prior to any scrimmages.

A copy of a notorized birth certificate and original for verification

Three copies of you child's FINAL report card from the 2009 - 2010 school year (does not apply to flag.)

No child will necessary paperwork completed, be permitted to participate in practice without having all of NO EXCEPTIONS.

Due to Pop Warner National Rules regarding conditioning time and contact time prior to scrimmages practice will begin on Sunday August 1st, @ 5:00 pm at Roggy Field.

Holmdel Pop Warner will also be hold our annual kickoff party that evening after practice....All Holmdel Pop Warner participants will receive food and drink.  The concession stand will be open for all who would like to enjoy some of HFA's famous bbq.
